Licensed Practical Nurse, LPN - Full-time/Per Diem (Home Health)

As an LPN, you will work under the supervision of registered nurses and physicians, delivering high-quality care that meets the physical, mental, and emotional needs of our patients. Your responsibilities will encompass a wide range of clinical tasks, from taking vital signs and administering medications to assisting in patient assessments and developing care plans. Responsibilities

• Provide direct patient care including monitoring vital signs and assessing patient conditions.
• Administer medications and treatments as prescribed by registered nurses or physicians.
• Assist in the development and updating of patient care plans based on individual needs.
• Maintain accurate patient records and documentation to ensure continuity of care.
• Support patients and their families with emotional and educational support regarding health conditions.
• Collaborate with healthcare team members to optimize patient outcomes and discharge planning.
• Ensure a clean and safe environment, adhering to infection control practices and safety standards.

• Graduate of an accredited school of professional nursing.
• Current license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the state(s) of employment.
• Minimum of one (1) year experience in an acute care setting or equivalent experience.
• CPR certified
• Demonstrated written, verbal, and interpersonal communication skills.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Has a good driving record.
