Clinical Care Coordinator - Full-time

We are seeking a dedicated and detail-oriented Clinical Care Coordinator to join our healthcare team. Responsibilities

• Coordinate patient care across various disciplines to optimize treatment and outcomes.
• Develop and implement individualized care plans based on patient assessments and needs.
• Facilitate communication among patients, families, and healthcare providers to ensure continuity of care.
• Monitor patient progress and maintain up-to-date records in accordance with healthcare regulations.
• Educate patients and families about treatment options, medications, and care procedures to empower informed decision-making.
• Assist in scheduling appointments, referrals, and follow-up visits for patients.
• Participate in quality improvement initiatives aimed at enhancing patient care and operational efficiency.

• Bachelor's degree in nursing, public health, or a related field.
• 2 years of experience as a Registered Nurse in a home health setting.
• Current nursing license (if applicable) or relevant certification in healthcare coordination.
• Proven experience in a clinical setting, preferably in care coordination or management.
• Strong communication and interpersonal skills to effectively engage with diverse patient populations.
• Ability to work collaboratively in a multidisciplinary team environment.
• Proficient in electronic health record (EHR) systems and Microsoft Office Suite.
• Strong organizational and time management skills to handle multiple responsibilities and priorities.
